A rich air.fuel mixture will give an O2 out of spec fault; however, the problem could be the IAC valve not working and making the fuel rich and giving an out of spec reading setting off the code. When I started actually paying close attention to it the needle was staying at the third little hash mark on the gauge (so technical right).
